Concrete contractors are typically responsible for providing a concrete structure that meets the specifications. To do this, they must match the needed concrete mixture proportions and properties with appropriate finishing equipment, tools, methods, and personnel. By using their experience with different concretes for different applications, they provide the owner with a concrete structure that meets or exceeds specification requirements, even in adverse cold and hot-weather environments and with challenging time schedules. Concrete contractors traditionally purchased concrete from the ready-mixed concrete producer. Now, however, more and more construction managers (CM) and general contractors (GC) are purchasing the concrete. The usual reason for this practice is economics. Some CMs and GCs believe that by purchasing the concrete they can reduce the project cost for the owner. The CMs or GCs also can benefit when they mark up the concrete cost. The markup will vary depending on the local market and how much coordination is required to interact, communicate, and make mixture adjustments throughout the concrete placement schedule. Concrete is not a commodity material. Concrete contractors use their expertise and judgment to evaluate proposed mixtures. This evaluation is especially important for mixtures to be used for floors that require hard-trowel finishes. When the CM or GC purchases concrete, it eliminates a critical review and evaluation procedure that is inherent when the concrete contractor purchases the mixture. When concrete has been selected solely on the basis of compressive strength and price, a contractors means and methods cant always compensate for mixtures with poor placeability and finishability. Concrete contractors can be solely responsible for meeting the project specifications only when they purchase the concrete. If another party purchases the concrete, there is a joint responsibility for uniformity of surface appearance and texture, flatness, occurrence of cracking, and other specification requirements. ASCC contractors want the responsibility for conformance with project specifications and for owner satisfaction. This responsibility includes input–before and during construction–that helps the concrete producer develop a delivery schedule and supply concrete with mixture proportions that satisfy requirements for both placing and finishing and hardened concrete properties. ASCC concrete contractors will accept this responsibility only if they purchase the concrete. ASCC concrete contractors will also cooperate with CMs or GCs who purchase concrete when there is a willingness to share responsibility for meeting project specifications.

iHealth, an online suite of medical
practice web sites and secure patient email services provided by Medem is
now integrated with Google Health, a new product that allows users to
store, organize and manage their medical records and personal health
information online. Medem is a healthcare IT company founded by leading
medical societies, including the American Medical Association (AMA).
The iHealth collaboration with Google allows Google Health users to share
information in their Google Health profile with doctors using the iHealth
web product. This in turn helps keep doctors up-to-date on current medical
conditions and medications. Physicians who offer the iHealth service to
their patients will be listed in the Google Health “Find a Doctor” feature
as integrated with Google Health.
“Improving communication and strengthening the patient-physician bond is a
key goal for Medem and its participating medical societies,” explained
Robert Musacchio, PhD, chairman of the board for Medem and senior vice
president, Publishing and Business Services for the AMA. “The collaboration
between Google and Medem to better connect patients and physicians online

MEMPHIS — NOFMA: The Wood Flooring Manufacturers Assn. released a technical information paper with its recommendation on the proper technique for gluing down NOFMA-certified solid wood floors directly to a slab. Previously, NOFMA did not recognize glue-down as a recommended installation procedure for solid wood, other than parquet flooring. With help from its technical committee, advancements in glues and the stringent manufacturing standards for NOFMA-certified wood flooring, the association has restated its position.
Introduction Markets for solid wood products have traditionally been dominated by the construction (70%), furniture (20%) and packaging sectors (5%). Over the last 15 years, the per-capita consumption of solid wood products has continued to decline. The erosion of traditional markets for solid wood is very evident. Building foundations, for example, are now dominated by concrete slabs, to the extent that many builders do not consider alternative pile or stub foundations and the traditional timber floor. Even when new homebuyers specify timber floors, there is now a tendency to float thin boards (often veneer flooring products) on the concrete slab to simulate the traditional floor. Technology is potentially available for designing solid (traditional) flooring which is durable, dimensionally stable and hardened. However, even with competitively priced products, it is difficult for the timber industry to compete with the now-established building practice. Preservative-treated wooden piles provide foundations that use mechanised production, are fast and simple to install, and durable. Pile foundations are arguably a better form of construction from an environmental perspective. They are also potentially more durable when considered from the perspective of a homeowners tendency to breach moisture barriers by building gardens against the veneer brick walls of the house. It will take promotion and advertising, possibly a generation of retraining of builders, and perhaps some form of alliance between builders and the timber industry, to regain the market for wood-based foundations. House framing is probably the single most expensive item in house building, representing about $15 000 for an average dwelling (A. deBruin, Auspine, SA, 2002, pers. comm.). In Australia, there is pressure to reduce the dimensions of timber used in traditional timber frames. But from an Australian forest industry perspective, consideration should possibly be given to adopting the sizes of North American framing systems (100 mm X 50 mm), so that domestic framing production is linked with potential export markets. The US has a decay and termite problem. New treatment technologies developed in Australia could give export framing an edge on the US market. Timber framing is under pressure in Australia. Steel is cheap to manufacture and streamlined in terms of size and quality, whereas timber is produced in a multiplicity of sizes and qualities. The building work force is also becoming more skilled in steel fabrication. Conversely, despite advances in pre-fabricating timber frames for delivery to building sites, there appears to be some resistance to such rationalisation. Traditional building practices of fabricating frames on site appears to be a preferred option. The use of steel purlins in roof construction is gaining in popularity and will potentially lead to further erosion of the traditional timber framing market. Interior joinery has largely been replaced with pre-painted moulded timber look-alike medium density fibre-board (MDF). Plastic is also making an inroad into this market. Even plaster mouldings have achieved a revival.

In the best of all worlds, a contractor who specializes in decorative concrete is very creative, possesses good knowledge of concrete, and has very good placing and finishing skills. But in the real world, one person rarely has all of these qualities. Sometimes people with creative talents in another medium get interested in concrete and start developing ideas. And, increasingly, concrete contractors are deciding to install decorative concrete because it increases what they can offer their customers. However it starts, the number of contractors involved in decorative concrete work is increasing rapidly. In recognition of this growing interest in creative decorative concrete, CONCRETE CONSTRUCTION and the Decorative Concrete Council (DCC), a council of the American Society of Concrete Contractors (ASCC), sponsored the second Artistry in Decorative Concrete demonstrations at the 2004 World of Concrete. Focusing on creativity, the goal was to show visitors to the demos the endless possibilities of decorative concrete and to encourage them to be more creative in their own efforts. The old adage “beauty is in the eye of the beholder” was especially true for the Artistry in Decorative Concrete demos this year. Everyone who came to see the finished work had their favorites, and there was considerable difference of opinion. But since the focus of the demos was creative expression and not selecting one artist as “the winner,” this event achieved its purpose. The following reviews of each of the 12 invited artists, highlights just how diverse the offerings were. Several years ago, Ehrenzweis, a professional artist, started experimenting with chemical stains on concrete and marble tabletops. Using solvent acrylic sealer as a stain-resistant material, he used fine brushes to paint elaborate, detailed drawings. Then he applied stain, which colored the exposed areas of the concrete but not the sealed areas. As his experience with chemical stains grew, his drawings became more complex by repeating the process of drawing with stain-resistant sealers and staining with different colors. This is the technique he demonstrated at WOC–showing many visitors how he achieved his remarkable patterns.

PHOENIX — Cosmetic surgeons are experiencing a major atrophy in business because of the national economic downturn. Both gross billings and patient volumes for cosmetic surgery were down by one-third in the past year, according to the American Academy of Cosmetic Surgerys 2008 Economic Impact Survey, released here during the groups annual meeting. “Times are tough, theres no doubt about that. Overall there has been a decline in most cosmetic practices, but I think some have been more affected than others. Practices that are more dermatologic-cosmetic and less invasive have seen much less if any decline. Some of the less invasive procedures have even seen an increase in some practices as patients who are trying to watch their dollars try to figure out what they can still do to enhance and improve their looks,” said Dr. Glaser, who is professor and vice chairman of dermatology at St. Louis University. In addition, many cosmetically-oriented dermatologists have been able to maintain a strong bottom line in the current economic climate by doing more noncosmetic procedures, she added in an interview.

Depending upon who is telling the story, the Endangered Species Act (ESA) (1) is either the “pit bull of environmental laws” (2) or a political fig leaf providing cover for business as usual. (3) Those differing views are mirrored by opinions about the scientific underpinnings of the Act and its implementation. Environmentalists insist that political considerations lead the United States Fish Wildlife Service and NOAA Fisheries, the two federal agencies responsible for implementing the ESA, to ignore science in favor of development, while the regulated community sees precisely the opposite problem. (4) Both supporters and opponents of the ESA are intent on gaining the scientific high ground. In part, that desire undoubtedly stems from the Act itself, which requires that a number of key decisions rest upon the best available scientific information. (5) But it goes deeper than that. Scientific victory seems to hold the key to political victory. Both sides seem convinced that public opinion turns on whether, and to what extent, science supports the decisions of the agencies responsible for implementing the ESA. There is good reason to think that this assumption is accurate. As a society, we hunger for objective, rule-based decision making, especially when the decision pits human interests against those of another species. We worry that decisions lacking a firm, objective basis may be arbitrary, wholly “political,” wholly dependent upon the whims of the particular decision maker, or made on the basis of improper motivations. We look to “science” to provide the objectivity we crave. The demand for strong scientific support of ESA decisions will only grow in the future, as the costs of conservation measures become higher and more apparent. (6) Science, however, is not as objective or neutral a basis for decisions as we might hope. In recent years, the uncertainties and gaps in the supposedly “scientific” decision making of the ESA have become increasingly apparent, and increasingly the source of controversy and contention. (7) At the same time, several reviews have strongly endorsed the use of science in making decisions under the ESA. (8) That juxtaposition suggests that different observers are looking for different things from the use of science in these decisions. The controversies raise important questions about how science is actually being used, what we are seeking from science, what we are actually getting from it, and what we should realistically expect.

A recent investigation was designed to see whether a phenomenon similar to naturally occurring relapse might be produced experimentally in a shorter time period by intentionally interrupting weight loss momentum during the initial treatment period. Investigators felt that artificially forcing weight stabilization might allow them to produce experimental relapses in a shorter time frame than occurs naturally during treatment. The purpose of the study was to experimentally test this disruption hypothesis by randomly assigning some patients to interventions that would disrupt their weight loss momentum. Subjects included 142 men and women (22 males and 120 females), aged 25 to 60 years with a mean BM1 of 33.1 [+ or -] 3.3 kg/[m.sup.2]. Subjects were randomly assigned to one of three groups that differed in the scheduling of the 14 group lessons. The control group completed 14 sessions, on 14 consecutive weeks. The long break group (LB) took a 6-week break after the seventh lesson. The short break group (SB) took 2 week breaks after the third, sixth, and ninth lessons. Participants in all groups were assessed at postreatment (week 20) and 6 months later (month 11). The three groups received the same behavioral weight loss intervention consisting of 14 group meetings. The subjects were also assigned a calorie goal of 1000 to 1500 calories per day depending on their baseline weight. Additionally, to create a formal rule for `break, participants were given a list of 13 high-fat foods that were to be totally restricted (for example, cheese, burgers, cakes, and pastries). Exercise was gradually increased up to 150 rain of activity per week. There were no group meetings during breaks and participants were instructed no to monitor their behaviors or weight during these breaks and to eat and exercise as they had before the program. Following the `breaks, treatment meetings began where they had left off. After completing the 14 lessons, all participants were encouraged to maintain their new eating and exercise habits and continue to lose weight or maintain their weight loss. The subjects appeared to take the breaks as prescribed, with interruptions in self-monitoring, dietary adherence, and self-weighing (but not exercise). However, participants quickly resumed their weight loss behaviors after the break. Breaks produced a slowing of weight reduction or slight regain, but weight losses during the breaks were not significantly different from the control group. Overall weight losses (0 to 5 months or 0 to 11 months) did not differ between the treatment groups.

Concrete contractors are typically responsible for providing a concrete structure that meets the specifications. To do this, they must match the needed concrete mixture proportions and properties with appropriate finishing equipment, tools, methods, and personnel. By using their experience with different concretes for different applications, they provide the owner with a concrete structure that meets or exceeds specification requirements, even in adverse cold and hot-weather environments and with challenging time schedules. Concrete contractors traditionally purchased concrete from the ready-mixed concrete producer. Now, however, more and more construction managers (CM) and general contractors (GC) are purchasing the concrete. The usual reason for this practice is economics. Some CMs and GCs believe that by purchasing the concrete they can reduce the project cost for the owner. The CMs or GCs also can benefit when they mark up the concrete cost. The markup will vary depending on the local market and how much coordination is required to interact, communicate, and make mixture adjustments throughout the concrete placement schedule. Concrete is not a commodity material. Concrete contractors use their expertise and judgment to evaluate proposed mixtures. This evaluation is especially important for mixtures to be used for floors that require hard-trowel finishes. When the CM or GC purchases concrete, it eliminates a critical review and evaluation procedure that is inherent when the concrete contractor purchases the mixture. When concrete has been selected solely on the basis of compressive strength and price, a contractors means and methods cant always compensate for mixtures with poor placeability and finishability. Concrete contractors can be solely responsible for meeting the project specifications only when they purchase the concrete. If another party purchases the concrete, there is a joint responsibility for uniformity of surface appearance and texture, flatness, occurrence of cracking, and other specification requirements. ASCC contractors want the responsibility for conformance with project specifications and for owner satisfaction. This responsibility includes input–before and during construction–that helps the concrete producer develop a delivery schedule and supply concrete with mixture proportions that satisfy requirements for both placing and finishing and hardened concrete properties. ASCC concrete contractors will accept this responsibility only if they purchase the concrete. ASCC concrete contractors will also cooperate with CMs or GCs who purchase concrete when there is a willingness to share responsibility for meeting project specifications.

Repeated evidence shows instruction by a registered dietitian (RD) is an effective weight loss strategy for various populations, different disease conditions, and in multiple settings. To determine how weight loss success due to an RD-led intervention compared with frequent weight monitoring, researchers examined a weight loss program that included RD-led weight-loss classes and frequent weigh-in visits. Investigators evaluated weight loss on a continuous scale based on attendance of both RD-led weight-loss classes and weigh-in visits that did not include RD interaction. They examined the relative strength of each of these factors on weight-loss success in a population recruited from the surrounding community for a hypocaloric weight-loss study. Eight of the participants for the hypocaloric weight loss intervention had type 1 diabetes mellitus, a condition that makes successful, safe weight loss especially difficult. The study subjects were overweight (BMI > 27.0) men (n=10) and women (n=16). At baseline the mean BMI was 33.2 to 3.67 and age was 43+12 years. Subjects with type 1 diabetes had mean baseline hemoglobin Ale (HbAlc) of 7.2% [+ or -] 1.0%. The energy prescription was determined as a 750-kcal deficit from estimated total energy expenditure. Subjects then participated in the 11-week weight-loss phase composed of 33 visits, including 11 weekly 1-hour RD-led weight loss classes and 22 semiweekly non-RD-related weigh-in visits. Weight loss (%) was evaluated on a continuous scale based on percent of RD and non-RD visits attended. Subjects with type 1 diabetes were asked to provide blood glucose records weekly and HbAlc was measured on the fist and last study visits. The 26 subjects completed the weight-loss program with an average weight loss of 3.8 [+ or -] 2.8 kg (P<0.001). There were no significant differences between sex or diabetes status in relation to weight loss, RD visit attendance, or non-RD weigh-in visit attendance. Among the subjects with type 1 diabetes, Hbalc was not statistically significantly different pre- versus post-weight loss. Weight loss (%) was significantly correlated with attendance of RD-led weight loss classes (r=0.451, P=0.021) but not weigh-in visit attendance that did not include RD-led weight loss classes (r=0.329,P=0.101). Frequent dietary instruction by an RD conveying topics related to weight loss was more effective than frequent weigh-in visits alone in promoting weight reduction in this study.
